Thank you for your generous donation to the Library’s Golden Horizons Campaign.
This form is for donors who have submitted a gift towards the Golden Horizons Campaign.
As part of the Golden Horizons Campaign, donors will receive a customized tile displayed in the Greenwood Heritage Library, as part of donor appreciation installation. This form is for donors to submit their preferred details for the tile customization associated with their gift.
Once the form is submitted, Library staff will be in touch with one final review of the customization details before the tile is produced.
Tile features include:
- Installation in the designated Greenwood Heritage Library window for 10 years
- Donor-selected recognition text*
- Accessible display location for easy viewing during in-person visits
- Tiered tiles are recognized by predetermined colours and sizes
- A visible and lasting presence in Greenwood Heritage Library’s legacy
* Custom text areas are subject to review and approval in accordance to library guidelines.
Tier Selection:
To support the Golden Horizons fundraising campaign, the available tiers are as follows:
- Sunrise – $300
- Dawn – $500
- Golden Horizon – $700 or more
- Silver Donor – $700 to $999
- Gold Donor – $1000 to $4999
- Platinum Donor $5000 or more
